Is this a bottle opener??

I picked this up at a thrift store this weekend, and I asked a guy walking by if he knew what it was. We both just kind of guessed it is a bottle opener, but then what is the second smaller grip area for?? Anyone ever seen this before? There are no markings on it, but it's a nice sturdy well made piece.
