How does this happen?
A planchet is fed into the die and struck. The coin sticks to the reverse die. Another planchet is fed into the die. The obverse is struck by the die; what would have been the reverse is struck by the obverse of the previous coin stuck to the die. This creates an incuse mirror image or a brockage. This piece sticks to the obverse die and is struck multiple times which "stretches" out the brockage. As it is continuously struck, the metal is pushed up the sides of the die creating a "bottle cap" or capped die. Extremely rare.
