This is the Tripoli Temple (Shriners) of Milwaukee WWI service medal. It is rare, the only one I have ever seen in 40 plus years. I am also sure that inspite of the small round loop this was issued as a fob based on similar looking fobs of the same organization. The Shriners are still very active and sponsor the National Shriner Childrens Hospital with proceeds from the Shrine Circus.

Thanks AR. I think you are on the right trail. I have seen a picture of a similar medal minus the WWI inscription on a ribbon watch fob below a design like your brooch. So it could be that your pin back was also part of a fob. In any event I have some members of the Tripoli Shrine in Milwaukee looking in to it for me.
