This is an electric (dry cell battery) clock. The clock is a spring driven mechanical wind-up clock. The battery operated the the light, bells and alarm. I believe this clock was made to be used in a hospital patients room.
It should have a bradded cord with a large wood push-button that would be connected to the back of the battery box. The connector on the left is marked LIGHT & the connector on the right is marked CALL. The switch on the top of the battery box when pushed all the way back towards the clock (as pictured here) would allow the bells to ring when the button is pushed. This would be used during daylight hours. When the switch is moved out, so it does not make contact on the bell connector, the light would come on when the button is pushed. This would be used during night hours.
This is just an assumption on my part because I do not have any material substantiating this theory, nor do I have the dry-cell battery or cord to test it out
