I have had this piece for awhile. I bought it at a local flea market. I knew it was Harrach; thanks to one of these guys. Can’t remember who ID'd it for me. Sorry:)
Anyway, seeing variations of it recently have made me look it again and appreciate it a bit more. I am assuming it would have the same form number 1734/2? What about the décor number?

it would have a different number system entirely but similar. Actually. I don't think form is about the shape it appears to have to do with year of manufacturer looking at the book. the decoration number is right but the 173x-x number is a production number. I am not really sure what that stands for precisely. there are three examples in the book and they all have different production numbers.. is it shape? dunno. the next page has two different decors in the same shape different size and their production number is 1887/10 and 1834/1 so it doesn't appear to be that.
Does yours have the ginko leaves?

I kind of think the decor number is referring to the enamelling. not the glass.
