My American Foursquare home was built in 1928 here in Brownsburg, Indiana. It’s 94 years old and needed a little tender loving care. I had a new roof put on the house and the garages, new guttering and gutter guard and last week I had everything painted. I think they did an outstanding job and it looks awesome and hopefully after I’m dead and gone it’ll last another 94 years. I’ve lived here for 27 years this July 31. It’s been a perfect home for my gas station memorabilia collection. Just wanted to show you all just how beautiful the yellow house on Green Street in Brownsburg, Indiana stands out. I’m very proud of this old house! They call me the Mayor of Green Street!! LOL If your ever my way stop and say hi.

I haven't conducted an exhaustive search, but I wasn't able to find the plan for your foursquare. There are more than a few out there for 2.5 storey foursquares with a single dormer and an off-center front door, but I couldn't find one with those two box bays.
Perhaps yours wasn't a kit?
