I've searched and searched online for this item, but with no success. I was wondering how much this would be worth, as well as what year it is from. I believe it is from the 1980's.

Menu sign form about 1978, that is when the Coke adds life theme was in use. Condition is good, but limited collectibility because of its recent vintage. But since it was actually produced by the company for use, it is considered Coca-Cola advertising, and would have some value to those collectors who collect items from this era. It would probably sell for around 50 dollars at a coke show.
It does have a bit of yellowing to it.. and no this one was not a service award. I got it out of a mom and pop store. Thanks for the info guys! I'll post the 36 year award soon.
Its in Petretti's Coca Cola collectibles price guide 11th edition (made in 2001) on page 256 listed for 45$. Ray and earlycoke were bang on with there prices. you are better off waiting 10-20 years and see what its worth than.
