I located this items in a small junk shop in New York City's South Bronx. Measuring 5 7/8 x 5 inches, the item appears to be made of stone. It could very well be a stone tile. I was told that the item was of the Romanesque period possible from Rome.
I welcome help in indetifying and translation of the text and symbols..........
