Original Swami Fortune Teller Napkin Holder made in the 1950's. Was made famous from being featured in the iconic Twilight Zone episode with William Shatner.

I don't this particular model was the one featured in the Twilight Zone episode with Bill, but same sort of thing. They had different kinds of mechanized napkin holders back in the day, and other devices including small jukeboxes.
