This is a circa 1950's carved wood figural bottle cork with a lever and spring action that when you press the lever the heads rise up, face each other, and kiss. - It is 5" tall (5 3/4" with lever pressed), hand-carved, hand-painted, and has "San Marinon" painted on in white on base part.
-- San Marino is an enclaved microstate surrounded by Italy, situated on the Italian Peninsula on the north-eastern side of the Apennine Mountains. Its size is just over 24 sq. miles with an estimated population of over 30,000.
