First name looks like Koehler or Kathleen.

I read the last name as Mulv.....
Thanks Newfld and kwqd! I ran some various searches and came up with KATHLEEN MULCAHY, a very accomplished glass artist. Mystery solved. Trying to contact her to ask her about the rest of the inscription.
