I was able to ID this as a Kellogg (by the patent dates, the last of which being an April 14, 1908, patent).

I've had the bell-box for about 8 months now, and now I have most of the candle-stick (the mouth-piece is missing). It looks great with the box.

Which is cheaper for replacement pieces, e-Bay or another site?

Any other way to nail a year on it? I opened up the face, fixed a loose wire, and could read a serial and "GM C" in it.

Your Kellogg candlestick was manufactured around 1912, the patent dates on the back cup was for the transmitter. The original transmitter was replaced with a newer
solid type transmitter, the mouthpiece and transmitter faceplate are molded into one.

Thank you for your help. It's amazing how much you know on telephones.

I was able to unscrew the speaker too, which provided more numbers, but no other patents.

It looks like you have a Kellogg mouth piece on your phone.
